From the West:
Travel State Highway 151 north to Madison. Just after the Verona exits, take a left (west) onto County Highway PD. Travel for about a mile to Maple Grove Drive, turn left and go about four blocks. St. Mary’s Care Center is on the left.

From the East:
Travel Interstate 94 west to Madison, then take the Interstate 90 exit south. At State Highways 12 & 18, exit going west. Take this to the Verona Road exit (State Highway 151) and travel south. When you reach the Highway PD intersection, turn right (West) onto County Highway PD. Travel for about a mile to Maple Grove Drive, turn left and go about four blocks. St. Mary’s Care Center is on the left.

From the North:
Travel Interstate 90/94 south. At the State Highway 12 & 18 exit, go west. Turn off at the Verona Road exit (State Highway 151) and travel south.
